Other REDCap Installation Supplement
The purpose of this supplemental form is to assist HSD staff in determining if the non- 91探花REDCap installation or other UW-managed REDCap installation (excluding 91探花ITHS REDCap and 91探花Medicine RIT REDCap Part 11) meets the 91探花criteria for security and reliability for conducting and documenting electronic consent (including compliance with 21 CFR 11 and with Washington State law).
Researchers wishing to use a non- 91探花REDCap installation complete this form.
