The paths depend on which IRB should conduct the review and whether the  91探花IRB should review on behalf of another institution.<\/p>\n<p><strong>These questions should be considered before the application is started and may require consultation with HSD. For example, the  91探花IRB does not generally review for the VA.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">Option 2:  91探花Reviewing for Other Institutions<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p>This pathway is most appropriate for research involving multiple institutions where the  91探花should review the activities of at least one other involved institution. In this pathway, the study approval is not approval for all sites. For definitions of Zipline terms:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.washington.edu\/research\/hsd\/training\/zipline-online-help-library\/zipline-glossary\/\">Zipline Glossary<\/a><\/li>\n<\/ul>\n<p><strong>The order of the application steps is flexible.<\/strong> You may find it better to create the study record in Zipline and then complete the Word-based IRB application form, especially if you plan to be a PI proxy for a study.<\/p>\n<div class=\"accordion \" id=\"accordion\">\n<div class=\"screen-reader-text\"><\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">Step 1: Set Up Zipline Access (One Time Only)<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p><strong>Everyone who needs access to the Zipline application must complete a one time self-registration process in Zipline before they can be added to the application.<\/strong> Each person must be added to the application before they are able to access it. This includes:<\/p>\n<ul>\n<li>The principal investigator<\/li>\n<li>Any study team members who will need to view or edit the application<\/li>\n<li>Faculty advisors (for student and medical resident applications)<\/li>\n<\/ul>\n<p>A  91探花NetID is required to register. Upload documents in Microsoft Word format. <a href=\"https:\/\/itconnect.uw.edu\/uware\/microsoft-office-for-desktop\/\" target=\"_blank\" rel=\"noopener\"> 91探花offers Microsoft Office<\/a> to  91探花students, faculty, and staff at no charge.\u00a0<\/strong><\/p>\n<ul>\n<li><a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/zipline-application-irb-protocol\/\">IRB Protocol Form<\/a>: Standard application form used to request review from HSD or the  91探花IRB.<\/li>\n<li><a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/zipline-application-irb-protocol-no-contact-with-subjects\/\">IRB Protocol Form, No Contact<\/a>: Shortened version of the standard IRB application form that may be used for studies that do not involve interaction of any kind (in person, email, internet, social media, etc.) with subjects.<\/li>\n<li><a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/external-irb-review\/\">External IRB Review Request Form<\/a>: Use this form to request authorization to obtain IRB review from a non- 91探花(\u201cexternal\u201d) IRB instead of the  91探花IRB.<\/li>\n<\/ul>\n<h4>IRB Protocol Tips<\/h4>\n<ul>\n<li>The IRB Protocol has sections based on topic. The first time you complete the form, scan the questions so that you know what is coming and don\u2019t duplicate information.<\/li>\n<li>Use the instructions and examples embedded within the form to help answer questions you may have about completing various sections.<\/li>\n<li>If you think that your research may qualify for a determination that it is exempt or is not human subjects research, only answer the questions labelled [DETERMINATION] on the IRB Protocol. If HSD staff determine that additional information is needed, they will ask you to complete additional questions.<\/li>\n<li>Reference other documents in the application, such as grant applications or study protocols, if they provide the needed information and are attached to the electronic application.<\/li>\n<li>The IRB Protocol is a living document that gets updated to provide complete and accurate information about your approved research activities.
