This is because of the longstanding and intertwined relationship between Bloodworks Northwest and  91探花hematology researchers, and because  91探花is often engaged in Bloodworks Northwest research.<\/p>\n<p>Bloodworks Northwest may optionally obtain review from the  91探花IRB for all research in which it is engaged, except research that:<\/p>\n<ul>\n<li>Is industry sponsored, or<\/li>\n<li>Requires the use of the Exception to Informed Consent mechanism, or<\/li>\n<li>Is cancer-related and involves more than minimal risk, or<\/li>\n<li>Involves Bloodworks Northwest as the lead institution for multi-institutional research for which Single IRB review is required except when all institutions are other local institutions with which  91探花has standing reliance agreements (e.g. Fred Hutch).<\/li>\n<\/ul>\n<h2>What to do if  91探花will do the review<\/h2>\n<ol>\n<li>Submit a <a href=\"https:\/\/www.washington.edu\/research\/hsd\/study-activities\/apply-for-review\/\"> 91探花IRB application<\/a> in the Zipline system. Be sure to:\n<ul>\n<li>Describe all involvement of Bloodworks Northwest in your application (the Zipline SmartForms as well as the IRB Protocol form that is uploaded).<\/li>\n<li>Follow the instructions for the <a href=\"https:\/\/www.washington.edu\/research\/hsd\/training\/zipline-online-help-library\/researcher-submission-guide\/submitting-new-studies\/application-paths\/#reviewingothers\"> 91探花Reviewing for Other Institutions<\/a> pathway.<\/li>\n<li>On the <strong>Study-Related Documents<\/strong> SmartForm: Upload a completed <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/supplement-multi-site-or-collaborative-research\/\">SUPPLEMENT Multi-site or Collaborative Research<\/a> to Question 3.<\/li>\n<\/ul>\n<\/li>\n<li>The  91探花IRB will review and approve the overall protocol. <strong><em>This is not yet approval for the involvement of Bloodworks Northwest<\/em><\/strong>. After the  91探花IRB has approved the overall protocol, HSD&#8217;s Reliance Team will work with you to approve the addition of Bloodworks Northwest to the  91探花application.<\/li>\n<li>After  91探花IRB approval for Bloodworks Northwest has been obtained, you are responsible for:\n<ul>\n<li>Informing Bloodworks Northwest of UW&#8217;s approval.<\/li>\n<li>Informing Bloodworks Northwest investigators about any conditions of the IRB approval.<\/li>\n<li>Informing Bloodworks Northwest investigators about their obligation to comply with  91探花IRB reporting requirements about noncompliance, complaints, problems, etc., although such reports must be submitted by you to the  91探花IRB.<\/li>\n<\/ul>\n<\/li>\n<li>Bloodworks Northwest investigators remain responsible for obtaining any applicable ancillary reviews from Bloodworks Northwest.
