This closes the SITE record while leaving the parent STUDY record open.<\/p>\n<p>To close the study once the closure criteria listed below have been met, follow the <a href=\"https:\/\/www.washington.edu\/research\/hsd\/training\/zipline-online-help-library\/managing-multi-site-studies\/#CR\">Multi-site Continuing Review or Closure instructions<\/a>.<\/td>\n<\/tr>\n<tr class=\"odd\">\n<td>External (Non-UW) IRB Review<\/td>\n<td>Contact <a href=\"mailto:hsdrely@uw.edu\">hsdrely@uw.edu<\/a> to initiate the study closure process.<\/td>\n<\/tr>\n<tr class=\"even\">\n<td>Exempt Determinations<\/td>\n<td>Closure is <strong>not<\/strong> required. If your research is completed and you wish to stop receiving automated email notices, email <a href=\"mailto:hsdinfo@uw.edu\">hsdinfo@uw.edu<\/a> to request that your exempt study be administratively data-parent=\"#accordion\" role=\"region\">\n<p>Criteria:<\/p>\n<ul>\n<li>All subjects have completed all study-related interventions and procedures, including any follow-up.<\/li>\n<li>You have obtained all private identifiable data and\/or specimens from all subjects.<\/li>\n<li>You have completed your analysis of all private identifiable data and specimens, as described in your IRB application.<\/li>\n<\/ul>\n<p><strong>OR<\/strong><\/p>\n<ul>\n<li>The study was never open for enrollment, no subjects were ever enrolled.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<div class=\"card\" id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p><strong>Identifiers do not need to be destroyed in order to close an IRB application.<\/strong> IRB approval does not have to be maintained during the records retention period if the study procedures and analyses described in the IRB application have been completed.<\/p>\n<p><strong>You can still provide information to subjects about study results or other studies if the activities were approved as part of the application.<\/strong> When former subjects are provided with information about other studies, the IRB review of those other studies must include review and approval of the information and recruiting efforts.<\/p>\n<p><strong>The  91探花IRB application can be closed if the study is permanently closed to enrollment at all sites under the  91探花IRB\u2019s review, all subjects under the  91探花IRB\u2019s oversight have completed all research-related interventions and interactions, and no additional identifiable private information or specimens are being obtained by the investigators under the  91探花IRB\u2019s oversight.<\/strong> This is true even if the  91探花study data have not yet been analyzed by the overall study statistical center\/personnel under the oversight of another IRB.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<h2 id=\"2\">Steps to Close Your IRB Approval<\/h2>\n<div class=\"accordion \" id=\"accordion\">\n<div class=\"screen-reader-text\"><\/div>\n<div class=\"card\">\n<div class=\"card-header\" applications only require the status report form<\/b>, but you have the option to upload other documents related to the continuing review.<\/p>\n<p><b>Any DSMB reports should be submitted separately using the <a href=\"https:\/\/www.washington.edu\/research\/hsd\/study-activities\/report-events-and-new-information\/\">reporting process<\/a>.\u00a0<\/b><\/p>\n<\/div>\n<\/div>\n<div height=\"337\" \/><\/p>\n<p>After the IRB has reviewed the application, the PI, any PI proxies, and primary contact will receive a notification that:<\/p>\n<ul>\n<li>The IRB has approved the closure; OR<\/li>\n<li>The IRB requires more information or a change before approving the closure<\/li>\n<\/ul>\n<p>Visit <a href=\"https:\/\/www.washington.edu\/research\/hsd\/training\/zipline-online-help-library\/respond-to-hsd\/\">Respond to HSD<\/a> for more information on submitting responses.<\/p>\n<p><b>After Closure<\/b><\/p>\n<p>After the continuing review is approved, the parent study will be updated. For closure requests, the parent study status changes to Closed.
